NY Flu Shot Clinic Reservations
Flu shots are free to all DGA members and dependents ages 13 and older, regardless of Health Plan coverage status. The DGA Foundation is generously covering the cost for members not covered under the Health Plan.
Reservation Instructions
To reserve a flu shot appointment, please complete the two steps below. Only one appointment can be booked at a time for each person in the family.
- Choose an available appointment slot.
- After submitting your appointment request, you will be automatically redirected to a second screen where you must complete a vaccine registration form. The registration form is REQUIRED to complete your reservation.
After you have completed your reservation, you will receive a confirmation email. Please keep this email in case you need to cancel or reschedule your appointment time. If you do not receive your confirmation email, be sure to check your spam folder.

TERMS OF USE: Please note this website requests and collects certain personally identifiable information voluntarily submitted by you for the limited purpose of scheduling flu shot appointments at clinics hosted by the DGA-Producer Health Plan, including your name, Plan ID number/DGA member ID, phone number, email, and participant type. By providing any personal information to this website, you fully understand and consent to use and sharing of such information by the DGA-Producer Health and any of its affiliates, partners, vendors or agents for the purpose for which such information was collected and provided. You are in control of, and solely responsible, for any personal information you provide to us online. The DGA-Producer Health Plan and any of its affiliates, partners, vendors or agents may make any information provided by you to law enforcement personnel or other third parties as required by law, or when such party reasonably believes it is necessary or appropriate to protect their or your rights or interests. Children are not permitted to use this website. This site may only be used or accessed by DGA members.
