New Anthem Blue Cross Health Plan ID Cards Coming Soon

Anthem Blue Cross will issue new Health Plan ID cards to all covered participants in the next few weeks. The new cards will reflect the updated out-of-pocket maximum limit for non-network claims, effective January 1, 2025.

A new card will be mailed to you and your covered spouse. Please destroy your old ID card once you receive your new card.

If you do not receive a new card or to request a replacement card, please contact the Health Plan office at (323) 866-2200, Ext. 401. Alternatively, an electronic copy of your ID card is accessible from the Anthem’s member portal at www.anthem.com.
