Weight Management Benefits Overview
Flyte Medical is the Health Plan’s new weight management benefit manager.
Beginning July 1, 2024, due to the significant costs of the new classes of weight loss drugs and to help ensure the Health Plan’s continuing support to sustain weight loss, any medications prescribed for weight loss on or after July 1, 2024 for participants and dependents ages 18 and older will be covered only if you are enrolled in the Health Plan’s new weight management benefits through Flyte Medical (“Flyte”), a medically supervised weight management program. For covered Health Plan participants and dependents age 18 and older, only weight loss medications prescribed by a Flyte physician will be covered by the Health Plan. If you are prescribed any of these drugs for treatment other than weight loss, the Health Plan will continue to cover your prescription as before and you are not required to enroll in Flyte.
Access to the Flyte weight management program is free to all covered Health Plan participants and dependents ages 18 and older who meet the program’s eligibility requirements. Services provided within program limits (see What’s Covered Under Weight Management Benefits? below for details) are not subject to co-payments or co-insurance. Prescription drugs prescribed by Flyte physicians, however, are subject to the Health Plan’s normal prescription co-payments.
Participants who are under 18 years of age are ineligible for Flyte and are allowed to continue treatment from non-Flyte providers. Weight loss medications will continue to be subject to Health Plan rules and prior authorization from CVS Caremark.
What Is Flyte?
Flyte is a medically supervised weight management program that goes beyond the scale. Experienced metabolic experts work with you to understand the unique aspects of your weight journey—zero judgment, 100% support.
The clinicians you work with at Flyte leverage a full spectrum of medications to tailor effective weight treatment based on your unique biology. You will work together with physicians, nurse practitioners, and registered dietitians who specialize in the treatment of obesity and support in sustaining weight loss, to help you manage your weight loss as well as related health conditions such as diabetes, heart disease, sleep apnea, and liver disease.
With Flyte, all treatment is delivered virtually through Flyte’s mobile app (Evolve), so you can conveniently connect with your providers and keep track of your progress.
Am I Eligible To Participate In Flyte?
To be eligible for Flyte, you must meet all of the following criteria:
- Be a participant or dependent currently covered by the DGA-Producer Health Plan;
- Be 18 years of age or older;
- Be physically located within the United States; and
- Have a BMI of 30 or higher or have a BMI of 27 or higher AND a weight-related health condition (such as diabetes, sleep apnea, cardiovascular disease, or liver disease).*
* Even if your current BMI does not meet the program eligibility criteria, Flyte may consider your historical BMI or past weight-related conditions in determining eligibility. You are encouraged to complete and submit the eligibility questionnaire by going to https://www.joinflyte.com/dgaplans and clicking Get Started.
How Does The Program Work?
Qualifying/Enrolling
- Go to https://www.joinflyte.com/dgaplans.
- Click Get Started.
- Answer a few questions to determine if you meet the eligibility requirements for the program.
- If you are eligible, you will receive an email with instructions for activating your account, including how to download the Evolve app through which you will connect with your Flyte care team.If you are not eligible, you will receive an email and/or phone call with further instructions.
Activating Your Account/Scheduling Your First Appointment
NOTE: First appointments can be scheduled as soon as July 1, 2024 (program launch date) and onward.
- The welcome email you receive upon qualifying for the program will include instructions for setting up your username and password for your Evolve account.
- After you have set your username and password, you will receive a second email with instructions for downloading the Evolve app.
- Sign into the Evolve app with your username and password to complete your onboarding checklist, which will include completing a Health Risk Assessment Questionnaire.
- About 1 to 2 weeks after you have completed your onboarding, you should receive an email and text message letting you know to schedule your first appointment with a Flyte provider.
- You will receive an email confirmation when your appointment has been scheduled.
- You will be sent a scale and blood pressure cuff once your first appointment is scheduled. These connected devices allow Flyte to track the progress of your weight loss and are required for use to participate in the program.
What’s Covered Under Weight Management Benefits?
During your first Flyte appointment, you will meet with a doctor who will create a personalized treatment plan based on your health history, genetics, lifestyle, and readiness to change. You will have scheduled visits with your dietitian and nurse practitioner to monitor your progress, troubleshoot any challenges, make modifications if necessary, and make sure you have everything you need to succeed. Weight loss medications prescribed by a non-Flyte provider are not covered unless you are under 18 years of age.
Your treatment plan will be as unique as you are, and it may include the following:
- Medications prescribed for weight loss, when clinically appropriate;
- Nutritional coaching with a registered dietitian;
- Goal setting;
- Weight tracking using a scale provided by and automatically connected to Flyte;
- Blood pressure tracking device provided by and automatically connected to Flyte;
- Up to seven virtual visits with your Flyte care team (four with a physician/nurse practitioner and three with a registered dietitian) during each consecutive 12-month period; and
- Educational courses related to health and weight management.
What Does This Program Cost Me?
The services described above, excluding medications, are free to those who enroll and are not subject to co-payments or co-insurance. However, prescription drugs prescribed by Flyte physicians are subject to the Health Plan’s normal prescription co-payments. Additional visits with the Flyte care team beyond the seven allotted visits—unless approved by a Flyte provider—are subject to pre-authorization by the Health Plan. If additional visits are not pre-authorized, you will be responsible for the following per visit fees, as periodically adjusted: $250 per physician visit, $213 per nurse practitioner, and $100 per registered dietitian visit.
